Job Overview
Main Purpose of the Post
1. To participate in the delivery of a flexible Home Care Service to rehabilitate, support and
maintain vulnerable people living in the community.
2. To deliver high quality intermediate care, and personal care services which promotes
rehabilitation in accordance with the personal service user plan, Choices Care Ltd Policy
and Procedures and National Care Standards. This may include assisting clients in and
out of bed to get dressed / undressed with personal hygiene including bathing. To
commode / toilet. Emptying commodes and disposing of incontinence pads. Practice
mobility and or daily living skills.
3. To enable service users to maximise their independence in accordance with the personal
service user plan.
4. To observe general behaviour, functional ability and response of client, record and report
information as requested to Domiciliary Team Co-ordinator / Care Manager using
documentation provided. This will be used to assist in the assessment of service users
needs.
5. To promote choice, privacy and dignity.
6. Develop rapport with clients to promote their interest and participation in activities.
7. To motivate clients to use and assist when trained and authorised by therapists with the
use of aids / appliances.
8. To liaise with multi-agency and family members involved in clients care.
9. To assist clients where necessary to facilitate supported home visits prior to
discharge from hospital or rehabilitation.
10. To undertake light domestic tasks, including laundry, making beds, where appropriate.
11. Following development of Social and Hospital Service Policy to assist clients to take
their medication in accordance with Pathway Care Services Policy and procedures and
National Care Standards.
13. To prompt and monitor clients to take their medication in accordance with Personal
Service Plan.
Requirements for Home Care Assistants
1. That you are able to demonstrate ‘Good Practice’ with regards to the care and support
of service users in the community.
2. That you are reliable, honest, have good time-keeping skills and can follow written
and verbal instructions.
3. That you will consent to a CRB/POVA (criminal records bureau) check.
4. That you can undertake the Service Requirements as detailed below under Main
Duties and Responsibilities.
Staff Training and Supervision
1. You will be required to be employed for a probationary period of 3 months
2. You will be required to be ‘shadowed’ by a supervisor on commencing the provision of
care in the community. The purpose of this is to monitor care worker competence and
to ensure that Care Assistants are comfortable with company procedures.
3. You will be required to attend regular supervision sessions with the manager and
to participate in the performance appraisal process.
4. In order to ensure and maintain a safe working environment, you will be required to
attend specific training sessions relating to Health and safety and Moving & Handling.
5. You will be required to attend ongoing training sessions.
